Make sure the de-capping pin is tight in the stem and the locking nut is tight on the stem in the die.. A loose pin can miss the primer hole and will break.

Topkat have you checked the rim of the brass that is not just dropping in the case gauge? If the rim has even a tiny ding/burr, it will do as you describe.
