Either loose or broken ejector plate or missing/broken extractor claw.. The mossberg bolt has 2 claws so it would likely be the one on the ejector plate side. Other than that, ensure youre using the proper length shells, i.e. nothing longer than 3" and also that youre cycling the action with enough authority for the ejector to kick the shells free of the action.
