Question Photo and Fingerprint card?

Hi,
I'm filling out my form 4 and all the associated paperwork and I have a couple of questions. First, what/how should I list as my reason for being fingerprinted (i.e. weapon transfer, NFA transfer, etc...). Second, I have made photographs of myself with a 2x2 in cutout that shows a frontal view from my collarbone/shoulder up. Will this suffice? Should I glue the photo to the form? Thanks,

'NFA transfer' should suffice. The photos sound fine, and there should be an instruction sheet on just how to attach the photos. I've always used clear tape.
