Default Suppressor Form?

Can someone tell me the forms or show me a link to the forms needed to purchase the stamp for legally owning a suppressor?

If you are gonna make your own, you need this. Costs 200 bucks.

If you're buying one from someone else, you need this. Also costs 200 bucks.

Make sure you print out the forms double sided or the ATF will reject them. You will also need form 5330.20 and finger print cards which you can order from http://www.atf.gov/forms/firearms/ You can order the form 1 or form 4 from this link also.

What do you plan on making or buying?

Also if you set up a trust, and will the suppressor to someone it negates the need to have the sheriff there.
